Vital Records Division
The vital records division is responsible for the maintaining, issuing and
recording of all births and deaths that occur in Fayette County.
The
Fayette County Health District has birth and death records on file from 1908 to
present. Birth
and death records are kept in the county of where the event occurred. You can come into the office Monday
through Friday between 8:00 a.m. and 5:30 p.m. to
obtain a certified copy of the desired certificate. We can process your request
while you wait or you can request the certified copy by mail. If you wish to make a request by mail, you must send a money order
or certified check in the amount of $17.00 per certified copy. Mail the request and money order
to:

Please allow 5-7 days for the return of your
certificate.
BIRTH CERTIFICATES
To obtain birth certificates from the Health Department, you may come to the
office or send your request by mail. If you need to make a request by
mail, we will need the name at birth, date of birth, father's name, and mother's
maiden name. A stamped , self-addressed envelope is required with payment.
If our office is not able to provide you
with the requested certificate, a private company, vitalcheck.com
in most cases, can provide you with a certified copy from most any area of the

DEATH CERTIFICATES
To obtain death certificates from the Health Department, you may come to the
office or send your request by mail. If you would like to make a request
by mail, we need the name of the decedent and date of death. A stamped,
self-addressed envelope is required with payment.
GENEALOGY
RESEARCH
We are glad to assist you in your family genealogy research, which can be done
either by mail or by coming in to our office. Written requests for
genealogy research must contain the name to be searched, type of event (birth or
death), approximate date of event, father's name, and mother's maiden name. Due
to recent changes in Ohio law we are no longer permitted to issue uncertified
birth or death certificates. Certified copies are $17.00 each. A
stamped, self-addressed envelope is required with payment. If you

COST
A certified copy of a birth or death certificate costs $17.00 each and is used
for obtaining social security benefits, survivor benefits, a work permit, a
driver's license, to apply for a passport or for children entering school.
RECORD LOCATION
Events (births/deaths) are recorded in the health district in which they occur.
