HOPE
for
Elderly Independence Program
Rental Assistance for
Frail Elderly
The HOPE program
targets low-income frail elderly families to provide rental
assistance. The coordinator will determine the eligibility
based on information provided during the interview. A social
service plan is created and
presented to the client for approval and signature. the
coordinator will make necessary contacts to agencies for coordination
of services.
The
supportive services may include personal care, case management,
transportation, meals, housekeeping, and other services essential for
maintaining independent living.
The client must be:
 | At least 62 years old
|
 | Income eligible
|
 | Deficient in three
Activities of Daily Living (ADL) which are as follows, but not limited
to:
 | Unable to prepare well balanced meals
|
 | Unable to drive to various places |
 | Unable to dress oneself |
 | Unable to clean housing unit or do laundry |
 | Unable to prepare paperwork such as
applications, medical forms, tax forms, etc...
|
